After-Action Review¶
Why It Matters¶
Experience without reflection isn't learning. Organizations that systematically review and incorporate lessons improve over iterations; others repeat mistakes.
The Key Move¶
After any significant action: what was intended? What actually happened? Why the difference? What should we do differently next time? Make it systematic and blameless.
Classic Application¶
US Army after-action review process. Every training exercise and operation reviewed. Lessons captured and incorporated into doctrine, training, and future operations.
Surprising Application¶
Project retrospectives. What was planned vs what happened? Not to assign blame but to improve process. The value is in the systematic habit, not any single review.
Failure Modes¶
- Blame focus (prevents honest assessment)
- No systematic process (learning is ad hoc)
- Lessons captured but not implemented
- Review too soon (immediate reaction) or too late (details forgotten)
